• Cost:  $20 per  and $10 for each additional w/in the family

VHSL PHYSICALS FOR 2010-11 SCHOOL YEAR

Front Royal, VA, -- On May 15, 2010, the Warren Memorial Hospital Medical Staff and employees in concert with the Warren County High School, Skyline High School, and Warren County Middle School Athletic Departments will be offering VHSL Physicals for the 2010-11 school year.  Physicals will be given at Warren Memorial Hospital--Same Day Surgery Unit. Any Warren County Public Schools student (7th grade through 12th grade) wishing to tryout or participate in middle school or high school interscholastic sports must have a physical completed on the VHSL form.  For the 2010-11 school year only physicals dated after May 1, 2010, are valid.  If you choose to have your child's physical completed elsewhere, it still must be on a VHSL form and be dated after May 1, 2010, to participate during the 2010-11 school year.  The cost is $20.00 for each Warren County Public Schools student-athlete ($10.00 for additional siblings).  Checks need to be made out to: Rising 8^th through 12^th graders of Skyline High School—please make your checks out to Skyline High School. Rising 7^th graders at Warren County Middle School and rising 8^th through 12^th graders of Warren County High School—please make your checks out to Warren County High School. All doctors and helpers are volunteers.  This is a fundraiser for the athletic departments. The money raised will be used toward our athletic training room expenses.

Time Schedule:

8:00 am to 8:45 am Rising 7^th, 8^th, and 9^th  grade girls

8:45 pm to 9:30 am Rising 10^th, 11^th, or 12^th grade girls

NO GIRLS WILL BE REGISTERED AFTER 9:15 AM

9:30 am to 10:15 am  Rising 7^th, 8^th, and 9^th  grade boys

10:15 am to 11:00 am Rising 10^th, 11^th, 12th grade boys

NO BOYS WILL BE REGISTERED AFTER 10:45 AM

Before you arrive Part I, II, IV, and V of the VHSL physical form must be filled out before you receive a physical. All parents' signatures are mandatory. No physicals will be given without properly completed and signed forms. You must have transportation available upon the completion of your physical. In addition, only show up during your scheduled time.
